AH-0181 Community HCA (Long Term Conditions)

30 hours per week

£9.89 per hour

We are looking for a dynamic and confident HCA who works well within a small team as well as able to work fully autonomously. The post is a new role within a well-established but small Home Visiting Team, which incorporates Paramedic Practitioners and Long Term Conditions (LTC) Practitioners. The role primarily will be that of supporting the LTC practitioners with housebound patients (including residential and nursing homes), supporting with phlebotomy, INR’s, QOF requirements as well as many other aspects to the role.

This is a developing role so is perfect for an individual with experience but also one who wants to develop their career and is motivated, who can come with innovative and exciting plans for future development of the service.
